Treatment Plan
The treatment plan of care letter must be written by a physician.
A pediatrician, developmental pediatrician, psychiatrist, or neurologist is able write a plan of care for your child. A psychologist is usually not able to write a plan of care.
It is important that your plan of care include certain keys words: Autism is a "neurological condition", your requested services are "medically necessary", and the treatment plan is labeled as a "treatment plan". In addition, the treatment plan should be on the physician's letterhead.
The following is an example for one to follow, however may need to be changed depending upon the individual child

The following treatment plan is medically necessary to treat this patient's neurological condition:
1)Applied Behavior Analysis/Applied Verbal Behavior Therapy, minimum of 20 hours of per week, up to 40 hours per week; (97532)*
2)Applied Behavior Analysis/Applied Verbal Behavior Consultation Services, minimum of 10 hours per month up to 20 hours per month; (96116)*
3)Speech therapy, minimum of one hour per week, up to three hours per week;
4)Occupational therapy, minimum of one hour per week;
5)Physical therapy, minimum of one hour per week;
6)SSRI medication as prescribed
